- • Applicants must be 18 years or older
- • Obtain medical clearance from a medical professional after last use
- • Meet criteria for the Residential level of care
- • Must be able to exit building in under 2 minutes unassisted
Mandatory items to bring with you to treatment:
All medications you are currently taking, in the original containers. (A 30 day supply)
A list of your medications, the dosage and how they are to be taken signed by your doctor (Including over the counter medications). We are required to have this per state law. Please note that we do not have a physician to write prescriptions.

Does Koinonia accept insurance?
As a courtesy, Koinonia staff will verify your insurance benefits. We highly recommend that you contact your insurance personally so you can fully understand your benefits. Insurance companies do not guarantee your benefits. We will work with you to provide information to best estimate your cost. Depending on the coverage, a payment may be required at admission.
Does Medicaid cover the cost of treatment at Koinonia?
Medicaid does not cover our treatment center as we do not have a 24 hour physician on staff.
